Adult Patients
To request copies of your medical records, please either call your clinic directly or visit My Health Connection to request copies online or through the app. With My Health Connection you can view your medical record and request copies.
Pediatric Patients
To request copies of your child’s medical records, please either call your clinic directly or visit My Chart to request your copies online or through the app.
